Diabetes

We have a team of Specialist Diabetes nurses who educate and support people with diabetes to help them control, understand and manage their diabetes and live as full a life as possible.

Note: the information below relates to the usual services we provide.

Home visits are available for the housebound. Diabetes specialist services are provided by Mid-Yorkshire Hospitals NHS Trust in north Kirklees.

Type One Diabetic patients may self-refer by calling 0300 304 5555. Type Two Diabetic patients will need to be referred via the GP.  Please discuss your query or concern with your GP who can assist you and then consider referral onto the Diabetes Nursing Team if required.

All Type One Diabetic patients are eligible for Continuous Glucose Monitors upon completing a 45-minute webinar. For more details, please speak to your Specialist Diabetes Nurse or your GP.
