What To Expect

First visit

At your first visit one of our District Nurses will carry out an assessment and work with you and your family or carer to discuss goals and agree a care plan and the frequency of future visits. Please have your discharge documents available and any dressings/equipment easily accessible. The team will also need access to a sink, liquid soap and paper towels/kitchen roll. If you have a hearing aid or other communication devices, please ensure they are available. If you need a carer/family member to be in attendance at your first visit, please contact the team to make them aware and a time can be agreed.


Ongoing visits

The team will carry out the care required as detailed in your care plan and review as necessary.


Visiting times

The nature of our work means we must prioritise patients who have specific timed visits and/or are in urgent need. This means we can’t give most patients a set time for their visit. It’s important you inform the team if you are attending any outpatient appointments and won’t be home on the day of your visit.  We provide a 24-hour service with core hours between 8am-5pm, between 5pm and 8am we carry out essential non-routine visits.


Discharge

You will be discharged from the team when your planned treatment is completed, you are able to self-care, you are able to attend clinics or appointments at your own GP practice or if your care is transferred to another service.


Working together

We expect our team to be able to work easily and safely in your home.

  • Please do not smoke in any rooms where nurses may be working, and no illegal drugs should be present
  • Pets should be under control when the nurses arrive at the front door and removed from the room during the visit
  • Ensure access to the property is safe from hazards
  • We have a zero-tolerance approach towards aggression, violence and verbal abuse

Your understanding is welcomed during harsh weather conditions when visits may need to be rearranged
