Services

Family Health Team

This service aims to actively engage and offer early intervention to families who have been identified as having additional needs and requiring an enhanced offer of support, antenatally through to the child’s 2-2.5 year check (as is required).

Parents to be will initially be offered a series of four 1 to 1 sessions to support Preparation for Parenthood, which will include the ante-natal contact. These sessions will:

  • focus on the health of the mother and unborn
  • ensure that every baby born in Kirklees has the best possible start in life
  • recognise and build on parents’ strengths and natural skills
  • promote confident, competent parenting that supports all-round child development
  • support family well-being in terms of stable relationships and enjoyment of parenting roles

Following the birth of the child, additional sessions will be offered according to need. The child and family will be transferred into the core 0-19 universal/targeted service at an appropriate time or following the 2-2.5 year check at the latest.
