Community Respiratory Service

The Community Respiratory Service is a multidisciplinary team who provide specialist care and support for people in Kirklees with chronic respiratory disease and their carers. Our patients are 18 years old and over.

We work in clinics alongside consultants to assess, treat, advise and support our patients. As a specialist service, we educate and advise general practices and other healthcare professionals involved in the care of patients with chronic respiratory disease.

In the Greater Huddersfield area, we provide a supported discharge service to support patients returning home from hospital following an acute illness. We also provide pulmonary rehabilitation. Pulmonary rehabilitation is an eight-week course that includes exercises and specialist education appropriate for people with chronic respiratory disease.

In North Kirklees the service is provided for patient with Chronic Obstructive Pulmonary Disease (COPD) only. 

We have links with the palliative care team, based at The Kirkwood, who provide end of life care and support for people and their families whose respiratory disease is in the advanced stages.

What we do

The service provides:

  • Assessment and diagnosis - investigating our patients’ symptoms
  • Self-Management - helping our patients to help themselves
  • Symptom management - monitoring and reviewing our patients’ symptoms and wellbeing
  • Education - making sure our patients and the health care professionals who care for them, understand chronic respiratory disease
  • Brief intervention smoking cessation - supporting our patients who want to stop smoking

What we don’t do

The Respiratory teams are unable to:

  • Carry out social care e.g. bathing, washing, dressing, cutting toe-nails
  • Provide a collection and delivery service e.g. medication and medical supplies

Please note that patients living in Calderdale with bronchiectasis are managed by the Calderdale Pulmonary Rehabilitation service. Patients living in Huddersfield with bronchiectasis are managed by the outpatient team at Calderdale and Huddersfield NHS Foundation Trust. 

Working together – what we expect from you

If you can’t attend an appointment – including a telephone appointment – or a visit to your home is no longer convenient, please let us know by ringing ‘Single Point of Contact’ on 0300 3045555. We can give the appointment to another patient.

We expect our team to be able to work easily and safely

  • If the appointment is in your home, please do not smoke in any rooms where nurses may be working for one hour prior to your visit and no illegal drugs should be present. Pets should be under control or removed from the room during the visit.
  • We have a zero-tolerance approach towards aggression, violence and verbal abuse
