Frequently Asked Questions

Going home

You will already have had discussions with the team about plans to return home. We will do everything we can to make this as seamless as we can. If you have any concerns please talk to your key worker or named nurse. Here are some questions and answers which may help you prepare.

Q: How will I get home?

A: If possible family members or friends should collect you and take you home.  If not, we can arrange appropriate transport.

Q: When will I know when I am going home?

A: We will try and give you as much notice as possible but sometimes it might be 1 or 2 days beforehand.

Q: What time of day will I leave?

On the day you go home we ask that you leave your room at 9.30am. You can stay in the lounge area and are welcome to stay for lunch or tea depending on the time of your transport home.

Q: Will I get help to pack my things?

A: Yes, the social care team will help you pack. If you have a lot of things in your room it might be worth planning how this will be transported back home. We can’t keep anything left behind longer than one week.

Q: What about my medications?

A: You will leave with a 2-week supply of medication. After that, you will receive your medication from your GP or any other arrangements you have in place.

Q: Does my GP know what’s happening?

A: When you leave we will update your patient notes which can be seen by your GP.
