The ITOC provide Nursing and Therapy support specifically to the Discharge To Assess beds, pre assessments for IMC, Reablement, and complex discharges. We support discharges from A&E and Frailty for therapy and falls assessments in the patient’s home to avoid admission.
We are unable to provide respite, long term package of care, benefit support but can signpost to relevant agencies such as Gateway to Care and Age UK.
