When a person is admitted to a hospital and resides in Kirklees, they may need support on discharge due to changes in mobility and ability to undertake activities of daily living.
If you are identified as requiring additional support on discharge by the hospital, our team is there to support this and prevent barriers and delays, most of this is behind the scenes, although you may get a visit from us in the hospital to assist with assessments for your next destination or extra support.
If you are transferred to a Discharge to Assess bed, if you have any nursing or Therapy needs you will meet our team who will undertake an assessment of your needs and set up care for your stay based on your needs.
