If you live in Kirklees, have been admitted to a hospital and need support when you go home due to changes in your mobility, our team will work with you to prevent any barriers and delays to getting home. You may get a visit from us in the hospital to assist with assessments to prepare for your next destination and to identify any extra support you may need.
If you are transferred to the Recovery Hub and require any therapy, you will meet our team who will undertake an assessment.
Any nursing care you need will be met by the Community Nursing Service
