Care Home Support Team

What to expect

As a new resident into the care home or on discharge from hospital the CHST may contact family and significant others following or before their assessment after obtaining permission from residents or as a best interest decision. We usually contact the person documented as first contact in care home or GP notes or person requested by resident.

This will be via:

  • Telephone in the first instance
  • E-mail
  • Through the care home when significant other visiting requesting, they contact the care home support team  

The resident/family will be supported by the care home support team to formulate an on-going plan of care with the focus on quality of life, future wishes and avoiding unnecessary admissions to hospital.
