Referral Criteria

In order for patients to be accepted into the service they must have a confirmed diagnosis of Heart Failure made by one of the following methods

Echo
Angiogram
Myoview scan

For your referral to be accepted you are required to be been known to the service in the past, if you are not previously know, we will require a referral from a relevant Health Professional.
