Please note: You will no longer receive an email from us to confirm we have received your referral - Once your referral has been received, it will be clinically triaged and a decision made about the possible treatment or care for you/the patient. If your referral is accepted, you will then be contacted by the appropriate team with the relevant information.
Referrals from hospital staff need to be received by 3pm for assessment and visits the following day.
If you are referring for a single need for catheter care or for trial without catheter please refer via the Catheter Hub / Home Catheter Care referral form.
If referring for multiple needs including catheter care, please submit this district nursing referral.
