Please complete the following form to book a place on one of our training courses (all questions marked with an asterisk are mandatory).

Please note: Your information will only be used by the Locala Sexual Health Team to confirm and complete your booking, and to enable the Team to send you course joining instructions or to contact you if there is a query relating to your application. It will not be used for any other purpose or shared with any other team or organisation. The information is held in line with Locala’s Information Governance policies and procedures.

Documentation needed for all training:

1. Certificate of Indemnity Insurance

2. Hepatitis B Status
3. DBS Clearance
4. Copy of CV
5. Evidence of recent Basic Life Support and Anaphylaxis training

Or alternatively we will accept a letter from your current employer or employing organisation’s human resources department confirming your employment status and fulfilment of pre-employment checks including all of the above.

In addition to the above for the following we will also need

For the Diploma upload:
Currently being updated in light of new diploma

For Coil Training upload:
- ESRH Module 18
- Evidence of EKA or Diploma

For the Implant Insertion Only upload:
- Contraception Online Counselling Course.
- ESRH Modules 8 & 17

For the Implant Insertion and Removals only:
- Evidence of EKA or Diploma
- ESRH Module 17

After booking is complete, The sexual health team will review all necessary documents and confirm your place via email. Payment will then need to be made before attending the course.
