All medical records requests are handled in accordance with General Data Protection Regulation (GDPR), the Data Protection Act 2018, the Access to Healthcare Records Act 1990 and rules of confidentiality.
During your home visit or clinical appointment you can ask your clinician to provide you with a copy of the entry or letter they are making into your medical record and your clinician will make arrangements for this entry or letter to be copied for you.
However, if you require your historical medical records it will be necessary for you to make this request to the Resolution Service. When the Resolution Service receives your medical record request, you will be asked to confirm your identity by providing a photographic Identity Document and a recent Utility Bill. Depending on the nature of the request, further documentation may be necessary.

We aim to complete a review within one month. In exceptional cases, we may require additional time to consider your case. In these cases we will provide you with an anticipated response date.
