End of Life Care
Palliative Care is end of life care for patients whose advanced progressive illness is not responsive to curative treatment.
Our team support clinical and care home staff to care for patients with cancer, dementia and other long term or chronic conditions enabling them to live well until they die.
Locala's specialist team of nurses and health professionals work together to provide care in the patient's home or care home working closely with Kirkwood Hospice. Treatments may involve managing symptoms, relaxation techniques, managing fatigue or providing equipment.
Our team provides support and encourages open discussion and planning between clinical/care home staff and patients, their families and carers during this period. At what can be a difficult time, our goal is to achieve the best quality of life possible for patients and their families.

We have a team of specialist Macmillan Nurses based at Holme Valley Hospital who provide education to staff in nursing homes in Huddersfield and North Kirklees and work alongside our District Nursing teams.

Referrals usually come from your GP or healthcare professional, however, patients can contact the service directly.
Kirkwood Hospice - A local hospice in Huddersfield who provide specialist care to people facing life threatening illnesses and support for their families and carers.
West Yorkshire Urgent Care Service - Urgent Care is for when you have minor accidents or unexpected health problems and need help within the next few hours. Urgent care services are not for life threatening problems or emergencies. If you have an emergency, always call 999.
National End of Life Care Programme - They work with health and social care services across all sectors in England to improve end of life care for adults.
Marie Curie Cancer Care - Their nurses provide care to terminally ill patients in their own homes or in our hospices, while offering support to their families.
Macmillan Cancer Support - They provide information and support to people diagnosed with cancer and their families.
Marie Curie Palliative Care Institute - A collaboration between Marie Curie Cancer Care, Royal Liverpool & Broadgreen University Hospitals NHS Trust and the University of Liverpool whose work is is directed towards making a difference to patient care.
Gold Standards Framework - It is concerned with helping people to live well until the end of life.
