Jubilee Rehabilitation Team

The Community Rehabilitation Team assesses, advises and provides rehabilitation for adults with physical disabilities. The team consists of a variety of Health Care Professionals including physiotherapists, occupational therapists, speech and language therapists and dietitians. Patients are referred to this service by their GP or other health care professional and are seen either at the team's base at the Eddercliffe Centre or the patient's home.
Jubilee Rehabilitation Team
The Jubilee Rehabilitation Team provides two services:
- A physiotherapy team treating patients 18 years and above, primarily within their homes, for example if there is a need for mobility assessments or mobility aids.
- A multi-disciplinary team consisting of nurses, occupational therapists and physiotherapists. The team assesses and treats patients over 65 years with complex needs such as multiple conditions, falls and Parkinson's disease. Patients are seen primarily within the Jublilee Centre, but may, on occasion be seen within their own home.
Currently, this service is only for patients who are registered with a GP and live in North Kirklees.
