Our Cardiac Rehabilitation team works with patients who are recovering from a heart attack or heart surgery and our Heart Failure team identifies and supports people at risk of chronic heart disease. 

Patients can refer themselves, or referrals come from GPs, practice nurses, district nurses, community matrons, therapy services, acute trusts, stop smoking team and specialist nurses.

Anyone in Kirklees aged between 40-74 who thinks they might be at risk can book a Health Check with their GP practice.

Cardiac Rehabilitation Team

The team work with patients following myocardial infarction, cardiac surgery, pre and post angioplasty or device therapy. With the support of the nurses, patients are to encouraged to alter their lifestyle, manage their condition and support self care.

The team of community based nurses work across the area with patients diagnosed with heart failure. They offer help and support to patients and their families co-ordinating care, monitoring and assessing the patient's condition and adjust medication when necessary.
