We work closely with those around us including GPs, hospitals and the Local Authority to ensure that the right care is given to patients in the right place, at the right time and by the right person.
We are committed to using new technology when we know it can make a difference to the care we provide. Last year we introduced Tough Books, a type of lap top that is updated at the point of care and Telehealth, a touch-screen system installed in a patients home allowing them to monitor their condition as well as automatically relaying that to a health professtional.
We are always looking at ways to enhance what we do and are currently developing Community Care Teams where patients are in the care of a 24-hour, 365 day a year, multi skilled team.
